Description of service/program:
Provides after-school tutoring and therapeutic activities for students as part of Care Team program.
Service limitations (income, geographic, hours, etc):
Serves 7th and 8th grade students ages 11 to 14 years old who attend Wadsworth City Schools, live in Wadsworth Township or any area of ZIP code 44274.
Hours:
Monday – Thursday: 2:45pm to 4:15pm during the school year.
Documents Required:
- Completed application form with parent’s signature.
- Referral from guidance counselor or teacher is required.
