Medical Services
Description of service/program:
General medical care provided for the working uninsured may include:
- Physical examinations
- Women’s health (annual pap testing, mammograms, pelvic exams)
- Referrals for diagnostic testing such as blood work and x-rays
- Referrals for physical and occupational therapy, dietician services, diabetes management and other specialties.
- Assistance with pharmaceutical patient assistance program
Service limitations (income, geographic, hours, etc):
To be eligible, patients must:
• Be age 18 and older (except for Youth Physicals)
• Live or work in Medina County
• Be within 250% of federal poverty guidelines (see Do I Qualify guidelines)
Clinic hours are by appointment only
