Senior Case Management
Description of service/program:
Senior Case Management is a free home-based program for seniors living in Medina County focused on helping each person achieve their best level of individual functioning as well as increasing their ability to succeed in the community and remain living in their home independently.
Program begins with an initial assessment of need to determine how to best serve each individual. After an assessment, individual services may include:
Case coordination
Symptom monitoring
At-home mental health counseling
Connection with community resources
Assistance with identifying and accessing services
Service limitations (income, geographic, hours, etc):
Serves seniors ages 60 and older who reside in Medina County. This program is provided at no cost to individuals.
